| 1. Excellent sportsmanship is expected at all tournaments and practices,
both on and off the court. Athletes will be warned of unsportsmanlike
conduct and a parent/guardian contacted. If the behavior continues, the
athlete may be removed from the team with no refund of club fees. Severe
behavior problems may result in the immediate removal from a team without the
refund of club fees.
2. Transportation will be the responsibility of the athlete and
parent/guardian participating in Club Digz. Coaches are unable to be
responsible for transportation to and from practice and/or tournaments.
3. Athletes are not guaranteed equal playing time; however, each athlete
will participate during tournament play. Our mission is to provide athletes
with greater volleyball experience while still being competitive in the USA
Volleyball Junior Club program.
4. Please provide prior notice when missing Club Digz activities:
Practice (4 hours before scheduled time) and Tournaments (24 hours before
scheduled time). This allows us to make needed adjustments. Not showing,
without prior notice, will result in a $25 fine. This fee will have to be
paid prior to the athlete returning to participate in any club activities.

All fees are put towards payment of gym space, equipment, individual
registration with USAVB, team registration with USAVB, tournament contract,
coaches, and uniforms.
6. All of the following forms must be turned in by Tuesday, December 2nd
prior to participation:
1.Individual Membership Form (both sides) – available on website
2.Medical History and Release Form (you do not need a physical) –
available on website
3.Copy of Birth Certificate (not the original)
4.Club Digz Guidelines Signature Form (just the page you sign) –
available on website
They may be mailed to the above address or given to Coach Heermans at school,
room 327.
